Jake DeWitt trained in obstetrics and gynecology — an unusual path for a primary care physician, and deliberately so. It gave him a clinical vocabulary that most family doctors don’t carry: hormones, reproductive health, surgical precision, the particular complexity of women’s physiology across decades of life.
He spent years inside two of Indiana’s most prominent practices — FreedomDoc and Westfield Premier Physicians — learning how care worked at scale. He saw what worked, what didn’t, and what was being quietly traded away in the name of efficiency. He saw physicians scheduled at 50 patients a day. He saw seven-minute appointments.
He saw what direct primary care could be, without the panel sizes and insurance intermediaries that turn medicine into triage. So he left, and opened Integrative Health DPC in Zionsville.
His direct line is his real cell. He reads every text himself. His appointments are 30 to 60 minutes. His panel is small enough that he actually knows his patients — and will still know them five years from now.

Integrative Health DPC is the only direct primary care practice in Zionsville offering BioTE® certified hormone pellet therapy inside the membership model. Dr. DeWitt is a BioTE Certified provider — a structured training and clinical framework built around subcutaneous hormone pellet optimization. Pellets are inserted in-office and typically last 3 to 5 months. For patients experiencing perimenopause, andropause, fatigue, or sleep disruption, this stays inside the practice, inside your relationship with your physician.
Women's Health.
Women's HealthDr. DeWitt completed an OBGYN residency before founding the practice — an unusual credential inside a DPC practice, and one that changes what he can do for women patients without a referral.
Routine annual care, hormone evaluation, complex menstrual and reproductive health conversations, perimenopause management: these don't get referred out by default. Karen Johnson brings her own clinical strengths to women's health as well. Together they run a practice where women's physiology across the full lifespan is treated as primary care, not a specialty referral.
Chronic Care.
Chronic DiseaseThyroid disease. Metabolic syndrome. Cardiometabolic risk. Diabetes management. These are the conditions that reward time — time to sit with a patient, understand their full history, track labs over years, and adjust accordingly. Karen Johnson holds a Doctor of Medical Science from Butler University — a post-professional doctorate that places her at the highest academic tier of physician associate practice. Her clinical training spans primary care and preventive medicine, with a patient philosophy grounded in the belief that good care starts with time.
She carries her own growing panel at Integrative Health DPC. That distinction matters: she is not a supplement to Dr. DeWitt’s practice. She is a second physician-level clinician running her own patient relationships inside the same direct-care model. Patients who join Karen’s panel get the same direct access, the same 30-to-60 minute visits, the same cell-level communication.
Her clinical focus includes chronic disease management, women’s health, and the complex metabolic picture that often takes years to fully address in a traditional practice — but can move faster when the clinician is actually available.
Together, Jake and Karen run a practice where the panel stays small enough that neither of them is racing the clock with you in the room.
